Background Despite a growing population of older transgender and non-binary (TNB) adults, evidence-based guidance for their care remains limited. There is a pressing need for a comprehensive research agenda developed in collaboration with, rather than solely about, this population.Aim This study aimed to co-create key areas and priorities for future research to improve quality of care for older TNB adults.Methods A four-day workshop was held with a group of 19 TNB individuals, clinicians, and academic researchers, representing diverse lived experiences, ages, occupations, and cultural backgrounds. A series of lived experience and scientific presentations were given and co-creation sessions were held to collaboratively explore the study’s aim. Concept mapping and design thinking principles were employed to generate and thematically organize insights during co-creation sessions. Data transcripts were then further analyzed using template analysis in consultation with co-creators.Results Three overarching themes were constructed. The first theme (reframing research best practice) offers guidance on future research conduct, emphasizing the importance of adopting participatory approaches, inclusivity, and the expansion of methodological perspectives to better capture complexity. The second theme (addressing knowledge gaps) highlights gaps in our understanding of gender-affirming medical care in older age and biopsychosocial influences on health. The third theme (practice-based research and knowledge mobilization) focuses on support for known challenges such as limited social support, best practice in elderly care, recognition and navigation of systemic power structures, and effective translation and dissemination of knowledge.Discussion This co-created agenda is intended to catalyze more responsive, inclusive, and equitable research and practice, ensuring that the voices and needs of older TNB adults remain central to efforts aimed at promoting healthy aging and quality of life in this population.
