This systematic review and meta-analysis evaluates the effects of supervised therapeutic exercise on cancer-related fatigue in breast and prostate cancer survivors. A total of 25 interventions from 19 randomized controlled trials (n > 2,200 participants) were analyzed. Supervised exercise significantly reduced fatigue compared to usual care (effect size = 0.34; 95% CI: 0.22–0.47). Subgroup analyses showed greater effects in men, patients with prostate cancer, and combined exercise modalities. These findings support the integration of supervised therapeutic exercise into comprehensive cancer care.
