Relatives refusals are the most frequent cause of loss of organs and tissues for transplantations. Methods: We made a statistical analysis of all reports to national transplantation bureau from 2017 – 2018. We find 46 families that refused to donate organs in 2017 – 2018. 31 family agreed to talk with us and we filled the questionnaire. Results: 60 % of families have never heard about donation before. 88 % have never spoken about donation. The primary reasons for families’ refusals to donate were: 40 % didn’t know the will of dead. 40 % did not understand brain death. 36 % did not believe health system. 32 % were afraid of loss of body integrity. 20 % were in shock. 16 % families were against donation. 8 % the dead was against donation. 4 % religious beliefs. Only 24% of the families were offered by doctors to donate organs as a rare opportunity to help other people. 52 % of families will change your decision after one year. 20 % still do not know. 20 % will not change, because they did not know the will of dead. 8 % will not change, because the dead was against. Conclusions: During the family approach the doctors should say that organ donation is a rare opportunity to help other people. Should be more positive educational programs for population. Not only donor cards are important, but also we should ask people to talk with their beloved ones about donation and transplantation. People in our country are not good informed about donation and transplantation. The sources of information can be TV, radio, and now we should not forget about internet and social media. Doctors who talks with families do not have enough knowledge how and when speak about donation.
