This review aimed to identify and analyze the main reasons related to non-adherence to HPV vaccination, as well as to synthesize the results obtained from reviews on the reasons for non-adherence to vaccination and on the safety of HPV vaccines. The reasons for non-adherence to HPV vaccination are multifaceted and include concerns about the safety and efficacy of the vaccine, lack of knowledge about the virus and its prevention, cultural and religious beliefs, cost, and limited access to healthcare services, among others. Lack of trust in health authorities and the pharmaceutical industry also plays a significant role in vaccine hesitancy. Reviews on the reasons for non-adherence to vaccination highlighted knowledge gaps, especially regarding the understanding of attitudes and perceptions of specific groups such as adolescents, parents, and healthcare professionals. Additionally, the safety of HPV vaccines was addressed, revealing a scarcity of information in some areas, such as long-term effects and the causal relationship with adverse medical conditions. Based on the available evidence, recommendations to improve adherence to HPV vaccination and enhance communication of risks and benefits associated with these vaccines include targeted educational strategies to increase knowledge about HPV and its prevention, addressing specific concerns about safety and efficacy. Additionally, it is crucial to establish transparent and accessible communication between healthcare professionals, health authorities, and the public, providing clear and evidence-based information about the benefits of vaccination and the risks associated with unvaccinated HPV infection. Efforts should also be directed towards improving access to vaccination by reducing financial and structural barriers. Collaboration between various sectors of society, including governments, healthcare professionals, educators, and communities, is essential to address the challenges related to non-adherence to HPV vaccination and ensure the protection of public health against this devastating viral infection.
