Epidemiology of Malaria in Hospital Intensive Care in Djibouti: Analysis of the Relationship between Plasmodium Infection and Hematological Parameters

Abstract

Malaria, according to data from the World Health Organization (WHO), generated 249 million cases and 608,000 deaths worldwide in 2023. Djibouti, like many other African countries, is impacted by this disease, in particular by Plasmodium falciparum, responsible for around 80% of cases, followed by P. vivax. Hematological and biochemical alterations occur in malaria-infected blood, leading to complications such as thrombocytopenia and disseminated intravascular coagulation. These elements are decisive in the diagnosis of malaria, thus highlighting the importance of analyzing their relevance in assessing the severity of the disease. This hospital-based study was conducted in two public health establishments in Djibouti, Peltier Hospital and Dar Al Hanan. It involved 149 patients, men and women, admitted for P. falciparum or vivax infection. Blood samples were taken from all consenting patients, and malaria infection was confirmed by rapid diagnostic tests (RDTs) and microscopic examinations, followed by analysis of hematological parameters. Results revealed a higher prevalence of P. falciparum among patients, with significant variations by pregnancy status among women. A significant correlation between Plasmodium infection and hematological parameters, in particular thrombocytes, was demonstrated. These results highlight the importance of maintaining malaria control strategies in Djibouti to eradicate this disease.
