Abstract

For the more than 90,000 patients currently awaiting a deceased donor kidney transplant in the United States,1 regulatory oversight of transplant programs has focused almost exclusively on post-transplant graft and patient survival — a framework that necessarily overlooked what happens to candidates before they ever reach the operating room. The introduction of two new Organ Procurement and Transplantation Network pretransplant performance metrics—the offer acceptance rate ratio (OARR),2 implemented in July 2023, and the pretransplant mortality rate ratio (PMRR),3 implemented in July 2024—represents a major shift in how transplant program accountability is conceptualized.
