ABSTRACT The infragluteal fold is an important aesthetic feature of the buttock and one of the primary concerns in reconstructive and contouring procedures. However, the detailed anatomical structure is poorly described, which may contribute to suboptimal surgical outcomes. This study aimed to re‐evaluate the anatomy of the infragluteal fold through gross anatomical and radiographic analysis. Five cadavers (10 infragluteal folds) were examined using gross anatomical dissection, and an additional five cadaveric specimens (10 infragluteal folds) were analyzed using soft‐tissue radiography to characterize the subcutaneous fatty and fascial structures associated with the infragluteal fold and their relationships to underlying anatomical landmarks. Gross dissection combined with radiographic assessment demonstrated that the fold is formed by two distinct fascial components: a bifurcated fascial structure anchored near the pubis close to the midline, and a band‐like fascial structure attached to the ischial tuberosity that anchors the dermis within the fold. These findings indicate that only specific segments of the fold are firmly tethered to underlying skeletal points rather than the entire length of the fold. This configuration creates a dynamic and complex three‐dimensional architecture in the gluteal region. A clearer understanding of this anatomical arrangement may improve surgical planning and reconstruction of the infragluteal fold.
