Vaccine confidence—the trust individuals place in the safety, efficacy, and importance of vaccines—is waning globally. Digital platforms, algorithmic content curation, and generative artificial intelligence have fundamentally altered how people encounter information and what they believe to be true. In this commentary, the authors argue that rebuilding vaccine confidence requires a strategic reorientation that centers on technology as both the source of risk and the site of potential remedy. By foregrounding platform design, algorithmic transparency, and AI governance, we can begin to imagine a future where digital technologies support, rather than subvert, public health.
