Cow’s milk allergy is the most common food allergy in early childhood. Extensively hydrolyzed formulas are commonly used as cow’s milk substitutes but they are not tolerated by all cow’s milk-allergic infants because of residual allergenic peptides or poor palatability. Other sources of mammalian milks have been then investigated. Because of a high cross-sensitization between ruminant milk proteins in children with cow’s milk allergy, goat’s and sheep’s milks should be eliminated from the diet of these patients. In contrast, equidae or camelidae milk, whose proteins display relatively low levels of sequence identity with bovine homologues, are generally well tolerated. However, cross-reactivity of equidae or camelidae milk with cow’s milk are still possible and specific allergy toward mare’s and camel’s milk protein have been also reported. Further clinical studies are needed to adapt and modify these alternative milks to meet the nutritional requirements of infants.
