Introduction: Vitiligo, traditionally considered a pigmentary autoimmune skin condition, is increasingly recognized for its paradoxical association with reduced cancer incidence, lower all-cause mortality, and enhanced immune surveillance. Emerging literature across epidemiology, immunology, and molecular biology suggests vitiligo may serve as a natural model of protective immunity. This scoping review aims to comprehensively map and synthesize existing evidence on the systemic protective associations of vitiligo, with particular focus on oncologic, transcriptomic, and immunologic dimensions. Methods and Analysis: This scoping review will follow the Arksey and O’Malley framework, refined by Levac et al., and guided by the PRISMA-ScR reporting checklist. A structured search will be conducted across PubMed, Web of Science, ScienceDirect, and Google Scholar, limited to English-language studies from January 2000 to March 2025. Two reviewers will independently screen studies and extract data using a piloted charting form. Extracted data will include study characteristics, outcomes related to cancer, mortality, comorbidities, and underlying molecular mechanisms. Ambiguities will be resolved by consensus or a third reviewer. Data will be synthesized thematically and presented in tables, figures, and narrative summaries. A PRISMA-ScR flow diagram will document the selection process. Ethics and Dissemination: As the review is based solely on publicly available data, no ethical approval is required. Findings will be disseminated through submission to a peer-reviewed journal, conference presentations, institutional seminars, and via the Open Science Framework (OSF).
