The aging processes lead to reduced appetite and the risk of becoming malnourished. Through the years malnutrition has not been acknowledged in the health care system, but it is common among elderly citizens aged 65 or older. This systematic review aim to investigate the evidence, and evaluate the effect of an individual nutritional plan combined with home visits for elderly patients after discharge from hospital, to prevent a reduction in nutritional status, readmission, and possible mortality for the elderly patients.
