This study aims to investigate the relationship between serum levels of human leukocyte antigen G (HLA-G) and renal transplant outcomes, exploring its influence on graft acceptance and immune-mediated rejection. Through a scoping review, we analyzed the available literature to understand how HLA-G functions as an immunomodulatory biomarker and the impact of different immunosuppressive regimens on its expression. Studies utilizing various methodologies, including ELISA, flow cytometry, and renal biopsy analyses, were considered to provide a comprehensive overview of its role in immune tolerance. Findings indicate that while some studies associate high HLA-G levels with prolonged graft survival, others highlight variations in its expression related to genetic factors, individual patient characteristics, and specific immunosuppressive therapies. The discrepancies in results underscore the need for further research to elucidate the molecular mechanisms involved and validate HLA-G as a reliable biomarker for rejection prediction. This study contributes to the understanding of HLA-G's role in renal transplantation and reinforces the importance of standardized protocols for evaluating immunological biomarkers in organ recipients.
