This scoping review seeks to meet two objectives: to evaluate the current literature on existing classifications of asynchronies and to propose a new classification (Adjustable, Innovative and Applicable in any ICU), with theoretical validity and consistency based on the literature and expert judgment, useful for the development of new research projects. Objectives Primary The objective of this systemic review with a scoping review extension is to systematically evaluate the current literature on existing classifications of asynchrony (based on respiratory mechanics, ventilatory curve morphology, electromyography/diaphragm [EM/D], and AI integration) and propose a new classification (Adjustable, Innovative, and Applicable in any ICU) with consistent theoretical validity based on the literature and expert judgment; feasible to apply in any ICU setting and adaptable to AI algorithms. Secondary • To assess the quality of the evidence collected. • To identify and map the existing types of PVA classifications and their main limitations. • To describe the characteristics of each classification: diagnostic methods, detection techniques, and validations. • To assess the strengths, limitations, knowledge gaps, and clinical applicability of the classifications
