Background: Modifying playing formats—such as the number of players involved and the numerical relationships between teams—during small-sided games (SSGs) is a common method to adjust the acute responses of players. Despite the extensive research available, there is a lack of a systematic review that consolidates the evidence in basketball, identifying the actual impacts of different playing formats on player performance. This information is crucial for coaches to make informed decisions when designing these games. Objectives: This systematic review with meta-analysis aims to compare the effects of smaller versus larger formats on basketball players’ physiological, physical, technical, and tactical responses during SSGs.
