Introduction: Hydatid cyst is a public health problem in endemic areas. It mainly affects the liver and lungs. Bone involvement is rare and represents only 0.5 to 2% of hydatid sites,and exceptionally concerns the iliac wing. The aim of our work was to study the clinical, radiological and therapeutic features of hydatid cyst of the iliac wing. Case report: We report a rare case of iliac wing hydatidosis in a 44-year-old female patient with no pathological history. She was admitted for low back pain. The diagnosis was made based on the findings of magnetic resonance imaging and the positive hydatid serology. The patient was treated with albendazole for 15 months with good progress. Conclusion: Hydatidosis of the iliac wing is rare. The clinical signs are nonspecific. The evolution is usually favorable under treatment.
